A coalition of community health associations is proposing the establishment of the "Discovery Island Community Health Centre (CHC)." We have provided a downloadable document that addresses frequently asked questions and offers information about the proposed CHC for the community. Additional information will be shared with our communities in the New Year.

Am I Eligible?
We encourage all interested individuals to apply. If you are unsure about your eligibility, please submit an application, and we will help you throughout the process. The following criteria will be considered at the discretion of the Board of Directors for applicants seeking housing in a Supported Living Residence:

Application
The date when the application is submitted.
Urgent Housing Needs
Preference is given to those needing suitable housing.
Income Requirements
Applicants must have a lower income (under $30,000) and rely on Old Age Pension, CPP, and possibly GIS. They should submit the latest Income Assessments for the past 2 years. Assets like property, savings, or investments are not considered.
Accessibility Needs
The applicant must require a wheelchair-accessible or adapted unit.
Age
Requirements
Applicants must be over 65 or have a partner/spouse over 65.
Residency and Citizenship
Applicants should be Canadian citizens or landed immigrants who have been residents of Quadra for over two years or have established family connections in the area.
Ability to Live Independently
Applicants must be capable of living independently and managing their daily living (ADLs), which include washing, dressing, taking medication, shopping, cooking, cleaning, and banking.
Competency in Management
Applicants should be competent in managing their financial, health, mental health, and social concerns, although they may need support with daily activities. Using mobility aids such as a cane, walker, wheelchair, or scooter is acceptable.
Applicants may qualify for the SAFER subsidy for rent. The SAFER program (Shelter Assistance for Elderly Renters) offers financial assistance to seniors, potentially covering up to $300 per month to help with rent costs.
